Introduction
The Washington County Community Services Department is soliciting proposals to identify and contract with an experienced organization(s) to provide Skilled Nursing Services to adults receiving Mental Health Targeted Case Management (MH-TCM) through Washington County Community Services Adult Services’ Division. Individuals who qualify for Mental Health Targeted Case Management must be diagnosed with a Serious and Persistent Mental Illness (SPMI), as well as additional criteria.
Individuals receiving MH-TCM are supported by Adult Services’ Division case managers to assess, coordinate, refer, and monitor medical, social, education, vocational, and other necessary services, as it relates to the individual’s mental health needs. Skilled Nursing is a valuable in-person service to enhance the effectiveness of MH-TCM and further support qualifying individuals in their homes and community.
Services in this competitive solicitation include: Skilled Nursing for individuals receiving MH-TCM (individuals diagnosed with SPMI).
The start date for services under this solicitation will begin on July 1, 2023, with a proposed contract term through December 31, 2024, and an option for annual renewal with a maximum total term of five (5) years (based on continued performance and funds available). These services will go through the RFP solicitation process every five (5) years, at minimum.
These instructions are intended to assist organizations interested in responding to the RFP. The entire application should be reviewed to ensure that all the requirements are understood.
This RFP does not commit Washington County to award a contract or to reimburse any costs incurred by successful or unsuccessful respondents in the preparation of their proposal. Washington County reserves the right to reject any or all proposals, to accept or reject any or all items in the proposal, or to award multiple contracts. Washington County reserves the right to negotiate with any respondent after proposals are reviewed, if such action is deemed to be in the best interest of Washington County.

General Submission Information
To be considered, providers must submit a proposal along with other supporting documentation in accordance with the instructions in this RFP. When evaluating a proposal, the review panel will consider how well the respondent has complied with these instructions and provided the required information. Washington County Community Services Department reserves the right to request clarifications from any respondent regarding information in their proposals.
Responses to this RFP should be economically prepared, with emphasis on completeness and clarity of content.
